An underground coal miner disables a continuous-mining machine’s emergency stop override so it can keep working while he is in its proximity-detected “shutdown” zone. He is fatally crushed by the machine.

An autonomous haul truck makes a turn that a water truck driver does not expect. The haul truck does not take evasive action and the two vehicles collide, causing damage to both.
